• The ACFE's 2026 study of 2,402 occupational fraud cases in 143 countries found the median scheme ran 12 months before detection. Frauds caught within six months had a median loss of US$40,000, against more than US$1.1 million for schemes that ran over five years, and controls such as management review and proactive data monitoring were associated with lower losses and faster detection.

• Actively monitored systems, where operators alert security or police while an incident is unfolding, were associated with significant crime reductions. Passive systems that only record showed no significant effect.
